ARTICLE 10 – ASSESSMENTS<\/u><\/strong><\/p>\n

Section 1: Death Assessments<\/u><\/strong><\/p>\n

  1. a)         Purpose:<\/u><\/strong><\/li>\n<\/ol>\n

    To assist in relieving the burden on a member or member’s family caused by any of the Deaths listed in this section.<\/p>\n

      \n
    1. b)         Application<\/u><\/strong><\/li>\n<\/ol>\n

      Beneficiaries are required to obtain a Death Assessment Application from the Secretary Treasurer and return the completed form within thirty (30) days of any such death.  A copy will be retained in the Local’s records.<\/p>\n

        \nParent of member                              $1.50 per member
        \nChild of member                                $2.50 per member
        \nSpouse of member                             $2.50 per member
        \nMember                                               $5.00 per member<\/p>\n
